3.8. Capsaicin Capsaicin, an alkaloid derived from chillies, depletes the neurotransmitter substance P from sensory nerves. Topically applied capsaicin cream showed a signicant effect in 3 of studies in diabetic neuropathy Chad et al., 1990; Schefer et al., 1991; Capsaicin Study Group, 1991; Tandan et al., 1992; Low et al., 1995 ; with NNTs in the positive studies from 2.5 to 4.9 and a combined NNT for all studies of 5.9 3.813 ; . Capsaicin has been tried with a positive outcome in postherpetic neuralgia Bernstein et al., 1989 ; and pain after nerve injury Watson and Evans, 1992 ; and the NNTs are 5.3 and 3.5, respectively. Although this treatment may have few side effects besides the burning pain on application at treatment start, it may be less convenient in many patients, since it has to be applied 4 times daily on the entire painful area. 4. Discussion The present review indicates that NNT to determine treatment efcacy in neuropathic pain provides consistent values for different drug classes. In painful diabetic neuropathy, the most extensively studied pathological condition, NNT values for tricyclic antidepressants and sodium channel blockers are around 23. This means that in neuropathic pain, 23 patients have to be treated before one patient with $ 50% pain relief is obtained. The benets and limitations of this simple approach will be discussed briey in the following see Fig. 1 ; . 4.1. Outcome measures A major problem in many clinical drug trials is the failure to predict outcome because individual trials often include a small sample size. In a recent study Moore et al., 1998 ; , it was shown that it is only possible to determine efcacy of a treatment from large trials. Thus, a meta-analysis of several trials may be necessary to generate a sufcient number of patients to determine the usefulness of a specic drug. With the use of a common outcome measure such as NNT-values, it is possible to compare and combine several studies. For pain, an essential parameter is obviously the degree of pain relief associated with a particular drug. From available studies, it is not always possible to determine a 50% pain relief and accordingly this gives rise to some uncertainty. However, it has been argued that since NNT describes the difference between treatment and control, a changing threshold for calculating NNT does not affect the result much McQuay and Moore, 1998 ; . An almost equally important aspect in determining drug efcacy is the potential harmfulness associated with a particular drug. In a nal analysis of a drug, it may therefore be essential also to determine the numbers needed to harm NNH ; , i.e. the numbers needed to treat before there is one patient with.

As with any drug, you must be extremely cautious if you are taking other types of medication for other conditions. Your doctors should be informed of every medication you have been prescribed. That caused problems. Some of the greatest damage was done by large projects supported by huge international development agencies. These organizations often seemed to have more resources than they knew what to do with and dumped them almost indiscriminately on hastily designed projects that had little local participation. And anywhere there was easy money, the opportunists would suddenly appear. I was on the verge of complete despair, but then I had the opportunity to visit several communities in other more remote parts of Haiti. I went to the Central Plateau, where I spent time with a group of Mennonite Central Committee MCC ; volunteers. They were living in very simple conditions with the people they were serving. The very efforts that the local feudal lords were trying to crush were the efforts the MCC volunteers supported. Progress was slow, and there weren't any large buildings to show off. But if you knew what to look for, the results of their work were truly impressive. With remarkably little money, they were under girding the efforts of several hundred farmers' groups. The farmers were learning techniques that allowed them to produce more food and reduce soil erosion. They were learning how to free themselves from the control of powerful speculators and middlemen who kept them from getting a fair price for their crops, while driving food prices high. The MCC volunteers were clearly working with rather than for the people. Shortly after this experience I visited the island of Lagonav and met Kathy Zimmerman, an American with Brethren Volunteer Service BVS ; . Kathy had lived for several years in a single room in one of the poorest and most remote parts of Haiti. With very limited funds, she was helping members of an association of community organizers develop a literacy program for women and men who had never had the chance to attend school. Kathy helped the instructors find training and materials for their students. No buildings were built for the classes, but what they lacked in facilities, they made up for in dedication. Classes gathered wherever they could--in a church, under a lean-to, beneath a shade tree with the chalkboard propped up against the trunk. The students pitched in to buy chalk for their teachers, and Kathy raised a little money to provide the instructors with more training and a small stipend. Together they were struggling forward on the arduous journey toward literacy and justice. Unlike the "orphanage" in my community, this literacy initiative clearly belonged to the people of the community. They knew the difference literacy could make in their lives. To them, to become literate was to feel human for the first time. No longer would they be put to shame. No longer would people take advantage of them and fool them with false contracts. They would no longer be ashamed in church when others read from the Bible or sang from their hymnals. Some among them would one day become teachers and even pastors themselves. I've heard it said that a cynic is a frustrated idealist. The simple idealism that had taken me to Haiti had soured into cynicism. But visiting these communities where Haitians and foreigners were living and working together in simple solidarity restored my idealism--an idealism now tempered with the realization that good intentions could never be enough. I have worked with a couple of shelters whose management has come out of the starting gate thinking that the lack of resources is their single greatest impediment to saving lives. That being said, one shelter had a budget of nearly million, about 30 paid staff and an annual intake of about 4, 000 animals. DESCRIPTION: TRANDATE Injection is an adrenergic receptor blocking agent that has both selective alpha1adrenergic and nonselective beta adrenergic receptor blocking actions in a single substance. Labetalol hydrochloride HCl ; is a racemate chemically designated as 2hydroxy5 1hydroxy 2 ; amino ; ethyl ; benzamide monohydrochloride. Labetalol HCl has the empirical formula C 19H24N 2O3 * HCl and a molecular weight of 364.9. It has two asymmetric centers and therefore exists as a molecular complex of two diastereoisomeric pairs. Dilevalol, the R, R' stereoisomer, makes up 25% of racemic labetalol. Labetalol HCl is a white or offwhite crystalline powder, soluble in water. TRANDATE Injection is a clear, colorless to light yellow, aqueous, sterile, isotonic solution for intravenous IV ; injection.
